On 7/27/2020 at 1:30 PM TO 3:30 PM, Licensing Program Analyst (LPA) Jose Calderon and LPA Ulysses Coronel iintiated a TA case management investigation for the listed facility. Due to the situation surrounding the Coronavirus Disease 2019(COVID-19), and to implement mitigation measures, today’s TA Visit was conducted telephonically with Terri Weitzman, the facility administrator.
Administrator showed LPA Calderon and LPA Coronel the common areas of the facility starting with the front door. Administrator then showed outside area to include smoking area.
Administrator states that all common areas will be open next week..
A telephonic exit interview was conducted with Terri Weitzman and a hard copy was provided via email for signature.
