From 11/18/2024 – 11/20/2024 LPA and North Los Angeles County Regional Center (NLACRC), Quality Assurance Specialist (QAS) Lisseth Carrillo corresponded via email and on 11/20/2024 at 3:00p.m. corresponded via video chat. On 05/06/2025 at 3:20 p.m. LPA conducted a telephonic interview with a family member / responsible person of a current client.
During today’s visit, starting at 10:08 a.m. LPA conducted the entrance interview and a brief physical plant tour to ensure there are no immediate health and safety concerns and facility is in compliance with Title 22 Regulations. At 11:57 a.m. conducted an in-person interview with Staff #2 (S2), at 12:12 p.m. conducted a telephonic interview with Staff #1 (S1), and collected copies of pertinent documents relevant to the investigation.
On the allegation Staff physically pulled R1’s fingers out of mouth it is the concern of the reporting party (RP) that facility staff including S1, and S2 pulled R1’s fingers out of their mouth. To investigate this complaint, LPA conducted in person interviews with three (3) staff and a telephonic interview with Administrator, a file review along with obtained copies of pertinent documents relevant to the investigation. From 11/18/2024 – 11/20/2024 LPA and NLACRC, QAS Lisseth Carrillo corresponded via email, on 11/20/2024 at 3:00p.m. corresponded via video chat. On 05/06/2025 at 3:20 p.m. LPA conducted a telephonic interview with a family member / responsible person of a current client. On 05/30/2025 at 11:57 a.m. conducted an in-person interview with S2 at 12:12 p.m. conducted a telephonic interview with S1 and collected copies of pertinent documents relevant to the investigation. Interview with S1 revealed that they formally worked at the facility as a Direct Support Professional (DSP) and worked 1:1 with R1 typically from 2:30 p.m. – 10:30 p.m. They never physically pulled R1’s fingers out of their mouth. S1 would verbally prompt and or verbally redirect R1 and R1 would listen. S1 would tell R1 “No, R1, fingers don’t go in mouth” and R1 would listen. At times S1 would introduce stimuli for R1 to redirect their fingers out their mouth and was very successful. S1 never saw any of the other staff physically pull R1’s fingers out of their mouth. Interview with S2 revealed that they currently work at the facility as a Direct Support Professional (DSP) and worked 1:1 with R1 typically from 6:30 a.m. – 2:30 p.m. They never physically pulled R1’s fingers out of their mouth. S2 would verbally prompt and or verbally redirect R1 and R1 would listen. S2 would say “No, R1, no fingers in mouth” and would redirect R1 with stimuli such as a toy. S2 never saw any of the other staff physically pull R1’s fingers out of their mouth. Interviews with staff revealed that they never saw S1 or S2 physically redirect R1. R1 was always verbally redirected by facility staff including S1 and S2. R1 was successful at following staffs verbal prompts and verbal redirection to take their fingers out their mouth.
