LPA Lopez made subsequent unannounced visit to investigate the above allegations. LPA met with front desk receptionist Olga Mejia and explained the purpose of the visit. Administrator Brandie Mendibles arrived a short time later and assisted with the investigation.

On initial visit on 11/08/2022 LPA conducted interviews with residents 1-10 (R#1 -R#10) Administrator Michelle Goodney and 1 facility staff (S#1-S#2). LPA obtained and reviewed staff and resident rosters.

On subsequent visit on 7/6/2023, LPA interviewed 6 additional staff members Including new Administrator (S#3-S#8) 2 additional residents. (R#11-R#12) and reviewed and obtained updated rosters of staff and residents.
Allegation: Staff speak inappropriately to residents in care. It is alleged that staff (Administrator Michelle Goodney) speak inappropriately to residents in care. LPA interviewed twelve residents and eight of twelve residents could not collaborate the allegations. (Continued on (9099C)

Several residents stated that all staff including Administrator Michelle Goodney are very nice and polite, and some residents stated that all staff are nice to them all the time. Some residents stated they are very happy here. LPA interviewed eight staff and all eight staff denied the allegations including Administrator Michelle Goodney and current Administrator Brandie Mendibles. All staff stated that they are always polite and respectful with all residents and have never heard of this allegation or had any residents complaint to them about any staff speaking inappropriately to residents.

Allegation: Staff not answering call buttons in a timely manner. It is alleged that staff take 30 minutes or more to answer call lights. LPA interviewed eight staff and all eight staff denied that it has ever taken more than 30 minutes to answer the call lights. Some staff stated that they are short staffed and answered call lights according to priority but never takes over 30 minutes. LPA interviewed twelve residents and nine of twelve could not collaborate the allegations. Some residents stated that the staff come right away or within 15 minutes. Several residents stated staff provide all their needs on timely basis. LPA randomly tested some call light respond time during the visit and the response time ranged from one minute forty-five seconds (1:45) to three minutes, forty-six seconds (3:46) for staff to respond.

Based on LPA's interviews, observations, the investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.



An exit interview was conducted with Administrator Brandie Mendibles and a copy of the report and appeal rights was provided.
