On 08/25/23 Licensing Program Analysts (LPA) Lizeth Villegas made an unannounced visit to the Cole Vocational Services San Pedro (ADP) located in San Pedro, Ca and met with Day Program Manager Tanisha Stewart as the purpose of today's visit was explained. The facility is licensed for a capacity of 30 adults ages 18-59, consumers are linked to the Harbor Regional center, the staff to consumer ratio is one (1) staff to three (3) consumers.

As a part of today's inspection LPA reviewed two (2) consumer records, two (2) staff records and inspected the physical plant. This is a day program located in one large building consisting of the following: Parking lot (drop off/pick up area), lobby area, there is a game area, style room, garden room, computer room, art room, relaxation room, a kitchen (knives are locked in Day Program Managers office), pantry, and storage room for toxins that are locked and inaccessible to consumers. There are administrative offices, four bathrooms ( 3 unisex and 1 women's), lockers and a small outside area. Facility walls and floors were in good condition, adequate lighting. Phones are operational, smoke alarms are hardwired and operational. LPA observed sufficient furniture and lighting throughout the facility, walkways throughout the day program and all exits were clear of hazards and debris. The facilities kitchen appears to be clean and well maintained, consumers bring their own meals and program provides snacks only.

The last disaster drill was conducted 07/19/23 and a Disaster Plan was on file. The last inspection held by the fire department was on 01/23/2023, four (4) Fire extinguishers were properly charged, and the first aid kit was available and fully stocked.

During today’s visit no discrepancies were observed. Exit interview conducted with Day Program Manager Tanisha Stewart, and a copy of this report was provided.
