Licensing Program Analyst (LPA), Joe Katrdzhyan conducted an unannounced site visit for the Required - 1 Year inspection. Upon arriving at the facility, LPA met with Program Director / Celeste Abernathy, who assisted with the visit. The facility is licensed as an Adult Day Program for adults between the ages of 18 - 59 years of age who are developmentally disabled, for a capacity of 60 consumers (40 Ambulatory and 20 Non-ambulatory clients only). There are 15 consumers attending the program today. Facility is located in a business complex. During today's visit, LPA used the infection control domain to complete the Required - 1 Year inspection.
The main entrance (front door) is used as the central entry point for all consumers, staff and visitors. A check-in station is set up by the entrance and routine symptom screening is initiated at entry.
LPA Conducted a facility walk through which consisted of a large building with 4 Activity Rooms, 2 Relaxation Rooms (of which one of the Relaxation rooms maybe used as an isolation room if needed), Conference Room, Computer Room and a Kitchen. All rooms have required furniture and equipment necessary to conduct all activities. COVID-19 signs are posted at the front door and throughout the facility. Documents are posted as mandated by Title 22 Regulations. Facility is equipped with a centralized sprinkler system. The fire extinguishers are fully charged and in compliance. Facility has four (4) bathrooms which were observed to be clean and operational. The hot water temperature was tested throughout the facility and measured within Title 22 Regulation guidelines. The first-aid kit is fully stocked w/First-Aid Manual, thermometer, scissors, tweezers, bandages and sterile first-aid dressing. Medications are centrally stored and locked inside the office. There are currently zero (0) clients taking medications at the day program. Knives, disinfectants and cleaning solutions are kept locked and inaccessible to clients.
Clients are expected to bring their lunch to the program daily. In the event that consumers come to the program without a lunch, Vocational Innovations will have petty cash available to provide them with a lunch. Snacks are provided to the consumers. Snacks are stored in the kitchen. A water fountain is available for the consumers.
Per Title 22 Regulations, there were no deficiencies observed during this visit.
An exit interview was conducted and a copy of this report was provided.
