| Operational Requirements: The facility has the appropriate fire clearance and conduct monthly emergency drills. Last drill conducted 03/09/26.
Staffing: The program maintains sufficient staffing during hours of operation, when clients are present.
Personnel Records Training: LPA reviewed five (5) staff files. Each file contained criminal record clearance, valid First Aid/CPR/AED certificates, health screening with TB clearance and sufficient training documented.
Client Records-Incident Reports: LPA reviewed five (5) client files. All files reviewed contained the following documents: Admission Agreements, Identification & Emergency Information, current Physician's Report with ambulatory status and TB clearance, Consent Forms, and Appraisal Needs & Services Plan.
Client Rights-Information: LPA observed the Client Rights and Reporting Posters, posted at entrance.
Food Service: Clients bring their lunches. There is a refrigerator with freezer available for storing.
Health Related Services: Clients in program do not have any incontinence care needs.
Incidental Medical & Dental: Staff do not assist with medication; clients do not take medications while at program.
Disaster Preparedness: There is a current Emergency Disaster Plan LIC 610D (12/21), last updated 01/16/26, with contact numbers and relocation sites.
Emergency Intervention: Clients at this facility have not needed the use of restraints or de-escalation techniques.
Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during today’s visit.
Exit interview was held and a copy of the report was provided to Administrator Reynaldo Aro.
