During the tour, LPA Bunker observed sanitizer, a visitor log, and a thermometer at the facility entrance. Logs documenting daily COVID-19 screenings and temperature checks for clients and staff were available and up to date. PPE supplies were readily accessible to staff, and an additional supply was observed on site. Adequate liquid soap, paper goods, and cleaning and disinfecting supplies were also noted. Staff were observed wearing face coverings and practicing social distancing.
All required postings were visible. Bedrooms contained the mandated furniture, and bathrooms were clean and operational. Personal accommodations were observed to ensure safety, privacy, and comfort, including non-slip mats. The kitchen was reviewed for its ability to safely prepare and serve food. Food service practices were appropriate, with an ample supply of perishable and nonperishable items stored properly. Resident medications were reviewed for proper storage, documentation, and system implementation; medications were locked, and records were current.
Common areas were clean, free of hazards, and suitable for resident use. The first aid kit was fully stocked, and smoke and carbon monoxide detectors were in compliance. Hot water measured 105 degrees Fahrenheit, within the required range of 105–120 degrees. The fire extinguisher was fully charged, linen supplies were adequate, and facility telephones were operational. Resident bedroom windows had no sliding window locks with thumbscrews, and all exit doors were in compliance. The yard was free of debris or hazards, and trash cans were properly covered. Staff have received training on dependent adult and elder abuse reporting.
There were no deficiencies cited.
