On 04/10/2026, at 9:15am, the department conducted an initial complaint visit to the facility and was greeted by Vashna Sar, Administrator, and Tawny Leslie, Manager. The department explained the purpose of this visit was to gather information about the complaint, gather facility files, interview staff and clients, and deliver findings for the allegations mentioned above.

The investigation consisted of the following: The department investigated the allegations mentioned in this complaint and conducted interviews with staff (S1-S4) and clients (C1-C3). The department received the following documents: Staff Roster (Dated: 12/01/2025), Client Roster (Dated: 01/16/2025), ID/Emergency Information (Dated: 04/10/2026, 02/22/2026, 12/07/2025), Functional Capability Assessment (Dated: 01/12/2024), Pre-Placement Appraisal (Dated: 01/12/2024, 12/07/2020), Physician’s Report (Dated: 01/04/2024), Individual Program Plan (Dated: 07/18/2024, 11/02/2023, 01/19/2024), Applied Behavioral Alternatives Quarterly Report (Dated: 10/02/2024, 03/02/2024), and Appraisal/Needs Service Plan (Dated: 07/27/2020, 12/07/2020) from the facility.

The investigation revealed the following: Allegation #1- Staff inappropriately touched client in care.

The details of the complaint alleged that the facility staff touched client inappropriately while in care. It was reported that staff often gave inappropriate hugs to client while at the facility. On 4/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff inappropriately touched client in care. All staff stated that they have never witnessed nor participated in any type of inappropriate behavior with any of the clients in the facility. Staff also stated that they have never touched, hugged, or did anything of an intimate nature that would cause any of the clients in the facility to feel uncomfortable with their actions.

The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that they have never been touched inappropriately or have been made to feel uncomfortable by any of the staff working at the facility.



Based on interviews conducted, there is insufficient evidence to support the allegation that Staff inappropriately touched client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation #2- Staff did not provide reasonable privacy to client in care.

The details of the complaint alleged that the facility staff did not provide reasonable privacy for clients while in care. It was reported that the facility had hidden cameras in the clients’ rooms. On 04/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff did not provide reasonable privacy to client in care. All staff stated that the facility does not and has never had hidden cameras in the clients’ rooms. Staff also stated that they are aware of the clients’ personal rights and have never impeded upon their privacy in any way.

The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that the facility staff does ensure that they have their privacy while at the facility and have never felt that they were being monitored in any way.



The department toured the facility and the clients’ bedrooms and did not observe any cameras or monitoring equipment in their rooms.

Based on observation and interviews conducted, there is insufficient evidence to support the allegation that Staff did not provide reasonable privacy to client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation #3- Staff denied food to client in care.

The details of the complaint alleged that the facility staff denied client food while living at the facility. On 04/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff denied food to client in care. All staff stated that they have never denied food to any of the clients past or present and that the facility gives the clients three meals per day plus any snacks that they would like.



The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that the staff has never denied them any meals at the facility and are satisfied with the care and supervision that staff gives them. They also stated that they get three meals per day plus snacks.

Based on interviews conducted, there is insufficient evidence to support the allegation that Staff denied food to client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation #4- Staff stole client's personal items.

The details of the complaint alleged that the facility staff stole clients’ personal items. It was reported that staff stole clients’ personal items that were of an intimate nature. On 04/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff stole client's personal items. All staff stated that they have never stolen any of the clients’ personal belongings, intimate or otherwise. Staff also stated that the facility has lock boxes for any of the clients to use if they have valuables or excess cash they want to protect.

The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that the facility staff have never stolen any personal items of theirs while they have been living at the facility.



Based on interviews conducted, there is insufficient evidence to support the allegation that Staff stole client's personal items. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

Allegation #5- Staff fed spoiled food to client in care.

The details of the complaint alleged that the facility staff provided spoiled food to clients in care while living at the facility. On 04/10/2026, from 9:30am-2:00pm, the department interviewed staff (S1-S4) and clients (C1-C3) regarding the allegation. 4 of 4 staff denied the allegation that Staff fed spoiled food to client in care.

All staff stated that the facility provides meals three times per day for the clients and has never served spoiled food to them intentionally or otherwise. They also stated that none of the client have ever told them that any of the food has been spoiled. They also stated that they do routine checks of the food items in the facility to make sure that any food items that are expiring are destroyed.

The department interviewed clients (C1-C3) about the allegation and 3 of 3 clients that were interviewed stated that the facility has never served them any spoiled food.



The department toured the pantry and refrigerator in the facility and did not observed any spoiled canned goods or other food items in the facility. All dates on all food items were current and not expired.

Based on interviews and observations, there is insufficient evidence to support the allegation that Staff fed spoiled food to client in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

An exit interview was conducted with Vashna Sar, Administrator, and a hard copy of this Complaint Investigation Report was provided.
