LPA Ramirez granted an extension to submit liability insurance no later than the end of business on 3/1/24.
LPA Ramirez will issue civil penalties in the amount of $700 for each deficiency noted above, 7 days x $100 starting from 2/17/24 till 2/23/24. Exit interview conducted with Caregiver Judith Sabando. A copy of this report and (2) LIC 421FC was provided.
