The investigation revealed the following: regarding the allegation “Staff handle residents roughly resulting in bruising.” It is alleged that staff# 8 (S8) handles residents in a rough manner. Seven (7) out of the nine (9) staff interviewed denied this allegation. Interview with staff# 3 (S3) revealed that they seen S8 be rough with resident#1 & 2 (R1 & R2). S3 revealed that R2’s family was a witness to S8 pulling R2 arm. Review of Unusual Incident/Injury Report dated 05/12/2026 revealed that R2’s family told S5 that they saw S8 pull R2’s arm. S5 reported the incident to S6. S6 asked R2’s family about the alleged incident and R2’s family denied telling S5 that they witnessed S8 pulling R2’s arm. Review of staffing notes dated 06/12/2026 revealed that staff asked R2’s family about the alleged incident but R2’s family denied making those allegations. R2 is no longer at the facility and was unavailable for an interview. LPA Ramirez made multiple attempts to contact R2’s family but all attempts were unsuccessful. Review of staffing notes for R2 did not corroborate this allegation. Review of Unusual Incident/Injury Report dated 06/05/2026 revealed that S3 reported that they witnessed S8 grab R1’s arms and twist them. S3 reported that S7 was a witness to the incident. Interview with S7 did not corroborate this allegation. Review of R1 staffing notes did not corroborate this allegation. Four (4) out of the four (4) residents interviewed denied this allegation. R1 and R2 were not at the facility during LPA’s visit and therefore not interviewed. LPA made attempts to contact R1 and R2 families, but all attempts were unsuccessful. Review of R1 & R2 medical records did not corroborate this allegation. Due to residents residing with cognitive impairments on this section of the facility, LPA was unable to conduct additional meaningful interviews. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

No deficiencies were cited during this complaint investigation. Exit interview was conducted with Administrator George Gonzalez. A copy of this report was provided.
