32 | Maintenance staff confirmed that the activation switch was not working properly early December due to it not being programmed, but started working again about 1.5 weeks ago. Both Assistant Administrator and maintenance staff indicated that the maintenance staff was trained to reprogram the switch if it is in disrepair. LPA tested the door switch during the visit and observed it to be working properly after maintenance fixed it today. 2 of the staff interviewed stated that the button to open the back door going out to the smoking area was not working for a few days last week. LPA interviewed a total of 5 residents, some wheelchair bound and some not. 4 out of the 5 residents indicated that the push button door switch was not working for about a week this month.
Based on interviews conducted, the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6 and Chapter 8), are being cited on the attached LIC 9099D.
An exit interview was conducted. The Plan of Corrections were reviewed and developed with Assistant Administrator. A copy of this report and appeal rights were provided. |
