25 | Licensing Program Analyst (LPA) Erik Zaragoza conducted an unannounced Required 1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA explained the purpose of the visit to Dana Ordonez, Administrator of the facility, and was granted access into the facility. There are forty-five (45) residents who currently reside in the facility.
The following 12 (CARE) tool domains were observed and reviewed: Infection Control, Physical Plant/Environment Safety, Operational Requirements, Staffing, Personnel Records/Staff Training, Resident Rights/Information, Resident Records/Incident Reports, Food Service, Planned Activities, Incident Medical and Dental, Disaster Preparedness, and Residents with Special Health Needs.
Infection Control:
· Infection control practices and Personal Protective Equipment (PPEs) were observed. Infection Control Plan will be submitted to LPA within seven (7) days.
Physical Plant/Environment Safety:
· The facility is a two-story building located in Los Angeles, California. It is licensed for a capacity of forty-five (45) residents, thirty-nine (39) of which may be non-ambulatory, twenty (20) may be receiving hospice, and six (6) of which may be bedridden, and the bedridden residents may only reside in rooms #101 – 106. During the inspection, it was revealed that a bedridden resident resides in room #212, which does not meet the facility’s fire clearance. It has twenty-four (24) residents rooms, a dining room, a kitchen, two (2) activity rooms, a laundry room in the basement area of the facility which is also where the locked cleaning supplies and chemicals are kept for the facility, and multiple bathrooms in client bedrooms, all eight (8) rooms that LPA inspected had a hot water temperature reading that measured between 105 and 120 degrees F.
· The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. |
