25 | Licensing Program Analyst, (LPA) Sanjay Vaid conducted an announced subsequent visit to this location, for the purpose of conducting a continuation of previous and partial visit conducted on 03/06/2024. On today's visit, LPA was greeted by Danna Gaytan, receptionist who allowed entry into the facility and Karla Portillo, Program Director. Administrator Gbenga Sowunmi was notified and met with LPA for the facility tour. The capacity of sixteen (16) clients, of which zero (0) may be non-ambulatory and zero (0) bedridden. Today’s census is 5(five) clients at the facility.
The physical plant and environment safety was conducted on 03/06/2024. Today’s visit was comprised of the following:
Linens & Hygiene Supplies: Required linen/supplies which include, pillowcase, fitted sheet, blankets, bedspreads. Mattress pads were observed. Emergency Phone Numbers, Exit Plan & Menu: Facility has a working phone landline. There are (2) cordless phone for client use. Fire Extinguisher observed. Food Service: All food and adequate utensils such as, dishes, cups, bowls and plates are stored at the other location until residents move in, all food is ordered and delivered for clients. Weekly menu on display. Appliances: Refrigerators, dish washer, laundry appliances, microwave, observed and operational. Toxins: Locked/stored for staff use only. Medications, First-Aid Kit & Book: Medications centrally stored and inaccessible to clients. First aid kit inspected which contains: thermometer, tweezers, scissors, antiseptic, bandages, gauze, which is available for staff use. Clients & Staff Files: Facility has a locked cabinet for client and staff files. Client and staff files were reviewed. Bodies of Water: None. Pets: None. Fire clearance: Fire clearance was approved on 9/21/22. Safety Drills: Fire drill was conducted 02/29/2024, earthquake drills are held annually.
No deficiencies were found on today's visit. Exit interview was conducted with administrator Gbenga Sowunmi. |
