25 | On 09/06/23, Licensing Program Analysts (LPAs) Denisia Jimenez and Claribel Soto conducted an unannounced case management inspection to discuss an incident report that was submitted to Community Care Licensing regarding an incident that occurred at the facility on 08/29/23. LPAs met with Director Patricia Logsden.
Patricia stated that on the date of the incident, Child #1 personal rights were violated by Child #2. Staff #1 was sitting at the table and looked up and saw the incident occur. Staff #1 walked over and checked and spoke with Child #1 and took Child #2 to the Director. Director contacted both parents and Child #2 was removed from the classroom and Child #1 did not return to the facility after the incident occurred. Through interviews conducted, it was confirmed that Child #2 did not have any similar incidents with other children or staff. LPAs reminded Director and staff to have visual supervision of children at all times whether they are outside or inside the classroom. There have been no further issues.
This appears to be an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted with Patricia Logsden. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days. |
