An Informal Conference was conducted on January 14, 2025, at 10:00 AM at the Fresno Regional Child Care Office. In attendance today was Licensing Program Manager (LPM), Juvenal Moctezuma, Licensing Program Analyst (LPA) Julio Rodriguez, and center Director Patricia Logsden. The purpose of the informal conference was to discuss previous deficiencies cited. LPM reviewed the deficiencies with facility representative and discussed how the Childcare Center (CCC) will maintain compliance with the deficiencies cited below.

The Department of Social Services, Community Care Licensing Division has determined that, Great Beginnings Learning Center has violated the following Title 22 licensing regulations/California Health and Safety Codes:

The following type A violations were discussed:

· 12/11/2024 CCR 101223(a)(3) Personal rights: Through evidence obtained, it was revealed that staff used inappropriate form of discipline by using spray bottles to redirect and or discipline children. This poses an immediate health, safety, or personal rights risk to persons in care.

The following Type B violations were discussed:

· 07/09/2024 HSC 1596.8662 (b)(1) Administration of Childcare Licensing: During file review, LPA observed two staff members did not have an updated mandated reporter training posing a potential health, safety, or personal rights risk to persons in care.

· 07/09/2024 CCR 101227 (a)(6) Food Service: LPA observed no menus or daily activity schedule posted which poses a potential health, safety, or personal rights risk to persons in care.

· 07/09/2024 HSC 1597.16 (a)(1) Lead Testing: During file review, LPA observed Licensee did not conduct lead testing by deadline which poses a potential health, safety, or personal rights risk to persons in care. Plan of correction is still pending.

·07/09/2024 CCR 101238 (a) Buildings and Grounds: During facility inspection, LPA observed several areas at the center needed to be dusted and cleaned. LPA also observed the carpet near the restroom was damaged and in need of repairs posing a potential health, safety, or personal rights risk to persons in care.

· 07/08/2024 CCR 101221(a) Child’s Records: During File review, LPA observed Licensee did not have a children roster on file which poses a potential health, safety, or personal rights risk to persons in care.

· 07/08/2024 HSC 1596.803(a)(1) Child Day Care Facility Licenses: Based on file review, LPA observed licensee had not paid annual licensing fees which poses a potential health, safety, or personal rights risk to persons in care.

· 06/18/2024 CCR 101238(a) Buildings and Grounds: LPA observed during a case management inspection that various areas of the facility needed maintenance which poses a potential health, safety, or personal rights risk to persons in care.

In addition, LPM also discussed the following pertaining to the dog that is accessible to the children in care:

· Licensee was directed to update her Plan of Operation - Center’s parent handbook, admission agreement, and staff job descriptions indicating that a dog is kept at the facility.
· Licensee was directed to indicate in handbook sanitary procedures for cleaning and disposing of dog waste.
· Licensee was directed to indicate in handbook that she accepts all liability in case of any accidents pertaining to the dog kept at the center.
· A copy of the Plan of Operation will be submitted to the Fresno RO for review no later than 02/14/2025.

In accordance with the regulations cited above, Licensee has agreed to the following:

1. Licensee agrees to comply with Title 22, Division 12, Chapter 1 of the California Code of regulations & The Health & Safety Code at all times.

2. Licensee understands that any form of discipline or punishment that violates a child's personal rights as specified in Section 101223 shall not be permitted regardless of authorized representative consent or authorization.

3. Licensee will ensure children are provided with a safe and healthful environment, and comfortable accommodations furnishing, and equipment at all times.

4. The Licensee will ensure that all staff, volunteers, and child files are complete and include the necessary LIC forms, training records, education, and experience documentation.

5. Licensee understands that she can contact Community Care Licensing, if she has any questions or concerns regarding the Operation of their Facility.

6. Licensee agrees to be referred to the Technical Support Program (TSP).

Licensee was informed that any further repeats of the above deficiencies will result in a Non-Compliance Conference and possible referral to the Legal Division for Administrative Action. Furthermore, Licensee understands that any repeats of the above deficiencies within 12 months will result in the issuance of civil penalties.

Licensee will stay in compliance with CCC Title 22, Division 12, Chapter 1 regulations at all times. A signed copy of this report and appeal rights were provided to Facility Representatives. No Deficiencies were cited during today’s Office Visit.
