On 5/25/23 Licensing Program Analyst (LPA) Denisia Jimenez and Licensing Program Manager (LPM) Juvenal Moctezuma arrived at facility to conduct an unannounced Annual Required Inspection. LPA and LPM met with Site Supervisor, Sylvia Sandoval and toured the facility. The facility consists of 3 preschool classrooms #2,3,4. This facility operates from 6:00am-4:00pm, Monday-Friday from May to November. LPA confirmed facility has a working telephone number of 559-416-5624. LPA observed 9 children in Classroom #3 with 2 teachers, & Classroom #4 with 3 children and 1 teacher and 1 instructional aide. Facility was operating within their ratio requirements. LPA observed all required postings. All individuals present have obtained a criminal record clearance.
LPA observed the playground area utilized by children is surrounded by appropriate fencing and the outdoor surfaces are safe for the children. While touring the outdoor play yard LPA observed multiple animal feces in the woodchips. LPA brought it to the Site Supervisor’s attention, and she immediately cleaned it up. LPA reminded licensee the importance of checking the centers surroundings before opening. Site Supervisor understood. LPA also observed tree branches hanging low which need to be trimmed so it doesn’t impede with children supervision. LPA observed that the outdoor equipment is age appropriate (2-5 years old) and in good condition. Shade is provided by a building overhang in the playground and 2 mature trees. Licensee stated that there are no bodies of water present in the facility, LPA did not observe any bodies of water. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts and all equipment appeared fine during the inspection. Licensee stated that there are no weapons on the premises. While touring the classrooms LPA observed in classroom 2 and 3 that toilet caps were missing. Licensee stated she has submitted a work order to get them replaced. The toilets and hand washing facilities are in safe and sanitary operating conditions.
During the inspection, LPA observed that Classroom #2 was used as a storage room. Site supervisor stated that they do not use it for children and that staff have access to it now. LPA reminded Site supervisor that this affects there Licensed capacity since that classroom was used to meet the capacity for 67 preschool children and cannot be used for other purposes. LPA discussed different options including a decrease of capacity if Site supervisor no longer wants to utilize that classroom. Sylvia stated that they will talk to their program director and let licensing know if they are decreasing their capacity or if they will be moving all their stuff out and keep their capacity of 67 preschool children by no later than June 1, 2023.

Food menus were reviewed, the Site supervisor stated they are posted at least one week in advance and posted for parents to see. Preschoolers are provided with Breakfast, Lunch and PM Snack. Food is brought over from the kitchen and children get to eat family style in their designated classrooms. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. Children are provided with drinking water and paper cups. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Facility has functioning smoke detector/carbon monoxide detector and a complete first aid kit.

LPA reviewed 5 children’s files. Children’s Records were reviewed for completeness and inspection of required forms was made. LPA reviewed 5 staff files. The center just opened May 8, 2023 so the children have not submitted their Physicians report yet LIC 701. Site supervisor understands that these need to be submitted within 30 days of enrollment. Sylvia stated that they will obtain these as soon as possible. Upon reviewing staff files, LPA observed that Staff #4 was missing a TB test. The rest of the files contain the required transcripts/verification of experience/immunization records, and Health Screening Report. All staff have current Mandated Reporter Training certificates. At least one staff has a current CPR and First Aid certification. Site Supervisor understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips).
The Site Supervisro was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The Site Supervisor states that the facility does administer medication to children. LPA reviewed form LIC 9166 for each child who is taking medication. Incidental Medical Services (IMS) policy was discussed. For IMS information, see PIN 22-02-CCP. any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill 2370, Chapter 676, Statutes of 2018 requires all Child Care Centers (CCCs) that are in buildings constructed before January 1, 2010 to test their drinking water for lead contamination by January 1, 2023 and every 5 years thereafter. This facility has provided evidence that their kitchen sink has been tested and has no exceedances.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
