25 | On 11/17/22 Licensing Program Analyst (LPA) Caroline Harris conducted an unannounced case management inspection. LPA met with director, Tina Jenkins to discuss an incident which occurred on 9/13/22. A census was taken and the LPA interviewed staff and observed the area in which the incident occurred.
On 9/13/22, at approximately 4:38 pm, three children were left alone and unsupervised in the main lobby at the front of the facility, for approximately three minutes. The director, Tina Jenkins saw the children when coming to the front. The children's parent was notified and Ms. Jenkins reported the incident to Community Care Licensing.
When interviewing staff, the LPA was also made aware that when the incident happened, two assistants/aides were supervising approximately 9 children by themselves, and there was not a qualified teacher in the room.
California Code of Regulations, Title 22, Division 12, Chapter 1, are being cited on the attached LIC 9099D. An exit interview was conducted with Tina Jenkins. A plan of correction was discussed, and appeal rights were explained. A printed copy of this report as well as a printed copy of the appeal rights was provided to Ms. Jenkins at the conclusion of the visit.
