25 | On 04/30/2026, Licensing Program Analyst (LPA) Jeovanna Yanez conducted an unannounced Case Management - Incident inspection for the infant license and was met by Director, Marcela Esparza. LPA toured the facility and a census was taken. The purpose of the inspection was to obtain additional information about an incident that was reported to Community Care Licensing (CCL) - Fresno Regional Office on 04/09/2026 regarding children's personal rights.
During today’s inspection, LPA interviewed staff, made observations at the facility, and reviewed video footage and records pertinent to the incident. Based on information received, on 04/09/2026 around 10:58 AM, it was reported that Staff #1 was assisting children wash their hands at the hand washing station inside the classroom when Staff #1 placed Child #1 down, and as Staff #1 was helping another child wash their hands, Child #1 began to walk towards Child #2, who was laying down on an activity mat, and Child #1 stepped on Child #2's right shoulder area. Right after this, Staff #1 is seen walking towards Child #1, moves Child #1 away from Child #2, and proceeds to hold and comfort Child #2. Child #2 was assessed by staff and no visible injuries were observed. Parent of Child #1 and Child #2' were notified of this incident.
Based on the information obtained during the inspection, LPA determined that no child’s personal rights were violated. LPA determined that the facility was within ratio and was practicing proper supervision during the time of the incident. Furthermore, this was an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiency is being cited: A copy of appeal rights was provided. An exit interview was conducted with Marcela Esparza.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
