On March 10, 2022, Licensing Program Analyst (LPA) Brannon, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with lead teacher, Maria Arreola, and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday from 8 AM to 1 PM. Lead teacher verified facility phone number is (559) 664-1109. Licensee is Community Action Partnership of Madera County.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. LPA observed and pointed out to Patricia Almanza, CAPMC Health Service Content Specialist, that there are several pieces of cabinet/shelving units that can tip over. LPA reviewed options on how to connect the furniture together to ensure the cabinet pieces do not tip. Per Ms. Almanza, licensee has the furniture connectors and will utilize them to ensure the furniture will not tip over.

Playground equipment is in safe condition, free of sharp, loose or pointed parts. During today’s inspection, LPA observed that the age appropriate sticker for the climbing structure is not posted. LPA informed lead teacher to request a replacement sticker for climbing equipment. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA reviewed with Ms. Almanza that the disposable cup dispensers are to be used to ensure children are drinking uncontaminated water. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. LPA observed wood chip is being used as cushioning. LPA observed that the area housing the climbing structure has adequate cushioning. LPA is recommending that staff rake the excess cushioning to fall zones to assist children from sustaining a serious injury from falls.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

Lead teacher, Maria Arreola, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Cottonwood Creek Head Start Center lead teacher is designated to act in the director’s absence and will be reported to the Department. A new lead teacher has been assigned and paperwork will be sent to Fresno Community Care office within two weeks.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training, which was completed on 2/11/22 for all three staff. Menus are posted at least one week in advance where an authorized representative can view them.

Fire drills are conducted monthly.

Currently, there are no children requiring Incidental Medical Services (IMS) attending this facility. Currently, licensee does not have an infant program. For future reference, LPA discussed the safe sleep regulations with lead teacher, Maria Arreola, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed lead teacher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA and lead teacher discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
