On 07/12/2024 Licensing Program Analyst (LPA), Stephanie Vega-Gonzalez conducted an unannounced Required Inspection and was met by Licensee, Maria Cosio. Licensee is Spanish Speaking and LPA assisted with interpretation. LPA introduced herself, explained the purpose of the inspection, and provided licensee with a copy of LPA’s business card. Days and hours of operation are Monday through Sunday from 1:00am to 11:30pm. Licensee does provide overnight care. LPA reviewed overnight care regulations. Licensee stated she understood.

LPA toured the home inside and outside and a census was taken. LPA reviewed current facility sketch and confirmed that the kitchen, bathroom, living roo, day care room, and front yard are accessible to children. All other rooms are off-limits and made inaccessible by door knob spinners. LPA reviewed PIN 23-11-CCP 6.19.2023 Large License Single Action Door Handle/ Single Action Lock. Licensee stated she will communicate with the department if she had any follow up questions.

LPA observed the following during the inspection. By the entrance hallway LPA observed the following: Grollia glue, nails, screws, 3-in-one Marksman oil, Whitening Strips, 4oz arthri-cream. In the living room LPA observed the following: A baby bouncer not in use, clear gorilla clue, Vicks Sinex, Diclofenac Sodium tropical gel, E6080 glue, FLANAX 24 tablet bottle, insulin needles in the box. LPA observed as licensee removed all the name items above and placed them in an inaccessible area in a kitchen cabinet. In the front yard LPA observed the following: clutter. Licensee stated that the front yard is currently not being used due to the summer heat. Licensee stated that she will clean the front yard before children are present or play in it. LPA reviewed with licensee items that are not permitted in a licensed family child care home. LPA printed an example sheet of what is and not permitted. Licensee removed the baby bouncer and placed it in an off-limit area and stated she will not use it for day care children. LPA did observe various roaches throughout the home. Licensee provided proof that she has hired an exterminator for the pest. LPA obtained a copy of the document.

There is no swimming pool or other bodies of water on the premises.

There are no firearms or ammunition on the premises. LPA observed a large aquarium with birds inside in the dinning room, a bird cage in the day care room, and a large aquarium in the day care room. LPA reviewed bodies of water regulation with licensee and advised supervising children around the aquarium and birds. Licensee understand the liability of pets around day care children and accepts responsibilities of any action taken by pets.

There is one fireplace in the home located in the living room and is made inaccessible by a cabinet and will not be in use during daycare hours. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. LPA advised licensee to do constant checks of their fire extinguisher and if she had any questions to reach out to her local fire department or fire marshal.

This is a single level home and there are no stairs. Safe toys and play equipment are observed. The home has working telephone service and LPA confirmed the phone number is (559) 673-1453.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. The outdoor play area in the front yard is fenced and there are no hazards to children present. Capacity as specified on the license is being maintained.

LPA reviewed a sample of children’s files and observed files were not complete with emergency information as required. Licensee and Assistant’s do not have a current Mandated Reporter Training. Licensee and Assistant’s do not have a current pediatric CPR/First Aid certification. Licensee stated that herself, assistant #1, and assistant #2 will complete the required training's. A review of records indicates that all employees and/or volunteers have immunization records on file for influenza, pertussis and measles.

LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to resources such as forms, regulations Provider Information Notices (PINs), and Quarterly Updates. LPA discussed Reporting Requirements as outlined in the regulations (Section 102416.2).

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with licensee During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA verified the RSO profile in FAS.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page).Licensee was provided appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
