On 12/17/2024Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted an unannounced Case Management inspection at facility. The purpose for the inspection was due to the facility being placed on the required visit list on 12/06/2022. LPA met with Licensee, Maria Cosio and explained the purpose of the inspection. Licensee is Spanish speaking and LPA translated throughout the inspection. Days and hours of operation are Monday through Sunday from 1:00am to 11:30pm. Licensee does provide overnight care. LPA reviewed overnight care regulations. Licensee stated she understood.

LPA toured the home inside and outside and a census was taken of three children in care. LPA reviewed current facility sketch and confirmed that the living room, dinning room, kitchen, day care room, bathroom, and front yard are used for providing care and are accessible to children. All other rooms are off-limits and made inaccessible by use of a door knob spinners and locked doors. During the inspection LPA observed the following: in the Entrance Hallway; LUCKY Body Cream 60z, TUMS container, Construction Screws in box 9x3, eye drops, and hair products. In the Living Room LPA observed; a dog pee pad with various pee stains. Children were observed playing in the living room around pee pad. LPA spoke to Licensee regarding the dog pee pad and licensee stated she will move in in the dinning area instead of the living room. LPA observed in the living room inside a coffee table a white bin full of various items: batteries of various sizes, an awl/hole starter, one scissor, a screw driver, and a electrical wire cutter. In the drawer under the television LPA observed a bottle of ADVIL and dog Heartgard Plus 0-25LBS chewable tabs, that was closed. In the Hallway LPA observed an cordless drill. LPA reviewed the importance of keeping hazards and dangerous items out of reach of children. LPA observed as licensee removed the items and made it inaccessible during the inspection. LPA observed a large fish tank in the dinning room being used for nine birds. LPA observed two large bird cages inside the day care room along with a fish tank. LPA observed two small dogs in the backyard, that children do not have access to. LPA observed one small dog inside the home. LPA reviewed best practices regarding the animals and children in care. LPA reviewed bodies of water regulations with licensee, who ensured that children do not have access to the fish tank and that children are always supervised. Licensee understand the liability of pets around day care children and accepts responsibilities of any action taken by pets.

There is no swimming pool on the premises. There are no firearms or ammunition on the premises. No poisons were observed during the inspection. There is one fireplace in the home located in the living room and is made inaccessible by a large cabinet and will not be in use during daycare hours. This is a single level home and there are no stairs.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. The outdoor play area in the backyard is fenced and there are no hazards to children present. Capacity as specified on the license is being maintained. LPA reviewed a sample of children’s files and observed that two out of three files were complete with emergency information as required. LPA observed that Child #1’s file was incomplete.

Licensee did not have proof of a Mandated Reporter Training. Licensee’s pediatric CPR/First Aid certification expires on 7/1/2025. A review of records indicates that all employees and/or volunteers have immunization records on file for influenza, pertussis and measles.

LPA discussed safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Exit interview conducted and report was reviewed with licensee. During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA verified the RSO profile in FAS. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency being cited: (see next page).

Licensee was provided appeal rights. This report shall be made available to the public upon request.

LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
