On 8/16/2024 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct an Annual Inspection. LPA was greeted by Case Manager Danielle Castaneda, LPA explained the reason for the visit, and Case Manager contacted Program Manager Zahira Soliz. Case Manager conducted the tour and once Program Manager arrived, she completed the tour with LPA.

Annual inspections were completed for both licensed facilities together #207206714 & #207206715. Facility #207206714 is licensed for a capacity of 60 and facility #207206715 is licensed for a capacity of 45, the current census for the program as a whole is 65. Clients are not kept in one building at a time and is split into different classrooms between the two buildings.

LPA toured both facilities and observed exits of both facilities to be clear & from obstructions. LPA observed both facilities/buildings to be clean, odor free and free from pests. During the tour LPA observed all chemicals to be locked and inaccessible to clients. LPA observed all storage area doors to be locked. Staff and clients were observed to be interacting with one another.

During the annual inspection Jorgensen Company was servicing the fire extinguishers. Carbon monoxide detectors was tested in each building and is in working condition. Water temperature was checked in each building. In building #207206714 the woman's bathroom was tested and read at 105 degrees F, and in #207206715 the water temperature was checked in the men's bathroom and read at 105.5 degrees F. Administrator provided verification of fire/earthquake drills recently completed.

LPA reviewed a sample of client's records which are current and up to date. A sample of staff files were also reviewed and are complete with verification of current training.

No citations were issued for either building. This report will also be entered under facility #207206715
