On January 15, 2026, at approximately 4:10PM, Licensing Program Analysts (LPAs) Nicole Tran and Brendon Van conducted a subsequent complaint inspection in response to the above complaint allegation. LPAs met with Site Supervisor, Ana Laura Zambrano Avila, and explained the purpose of the inspection. President and CEO of North Bay Children's Center, Susan Gilmore, later joined the visit. Present during the visit was 31 preschool age children.

As part of the investigation, interviews were conducted, and all relevant documents were collected and reviewed. Although the above allegation may have happened or is valid, based on LPAs' interviews, observations, and record review which were conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is determined to be UNSUBSTANTIATED at this time.

No deficiencies were issued today during LPAs' visit.

Notice of site visit was given and must remain posted for 30 days. Appeal rights were provided and explained.

Exit interview was conducted with facility representative, Susan Gilmore.
