On 7/25/2024, Licensing Program Analyst (LPA) Hanson Leong conducted an unannounced annual visit to the Family Childcare Home (FCCH) listed above. LPA met with the Licensee, Rina Santizo, and explained the purpose of the visit. At 12:54 p.m., the second licensee, Juan Alvarez, arrived home. According to Mr. Alvarez, he was at a doctor’s appointment.

Eleven children (three infants, six pre-k, and two school-aged) and five adults (the two licensees, the licensee’s son, A1, and A2) were present during the visit. The two licensees and the licensee’s son received criminal record clearance from the department. The licensee holds a large license and is within capacity limits and ratios.

LPA requested an ID card from A1, but A1 claimed that she did not have one with her. LPA also discovered A2 in the garage room. The licensee admitted to the LPA that A2 was not fingerprinted. LPA looked for A1 and A2 in the CDSS Guardian Background Check System and found that both A1 and A2 did not receive criminal record clearance.

The two licensees own the home and live with their four children (all over the age of 18). The hours of operation are Monday through Saturday from 6:00 a.m. to 8:00 p.m. One dog was present in the home. According to the two licensees, the dog is fully vaccinated.

LPA observed that all required documents, such as the facility license, the Notification of Parental Rights, and the Earthquake Preparedness Checklist, were displayed in a prominent, publicly accessible location.

The most recent emergency disaster drill was conducted on 7/7/2023, and the LPA observed that it was properly documented. LPA reviewed the emergency disaster drill log and found that they were not conducted every six months

Day-care Areas: Living Room, Family Room, Dining Room, Half Bathroom and Yard Area.

Off-limit areas: Laundry Room (pass-through only), Kitchen (pass-through only), and Entire Upstairs Area (Bedroom #2, Bedroom #3, Bedroom #4, Bathroom #1, Bathroom #2, Bathroom #3, and staircase). The licensee understands that off-limits areas may not be used for childcare during business hours.

LPA conducted inspections of the indoor and outdoor daycare areas to identify health and safety hazards. A fire extinguisher, a first aid kit, and multiple carbon monoxide and smoke detectors are present in the home. LPA found the home's interior to be clean and orderly, with proper heating and ventilation for safety and comfort. LPA observed multiple children going to get their diapers changed in the laundry room. The garage is located next to the laundry room. Disinfectants, cleaning solutions, poisons, and other hazardous items were accessible and stored in the garage without a child safety lock on the door. The bathroom was found to be clean and fully operational. The toilet and handwashing facility were well-kept, safe, and clean.

LPA observed that the daycare areas had age-appropriate toys, furniture, and educational materials. No equipment, such as walkers, bouncers, or similar objects, was present in the home. The fireplace was observed to be properly barricaded. Electrical outlets were observed to be properly covered with child safety covers. A child safety gate was observed at the stairs. According to the licensee, no firearms or weapons are on the premises.

The outdoor area was enclosed by a fence and equipped with play equipment for the children to use. The areas around and under high climbing equipment, swings, slides, and similar equipment were cushioned with material that absorbs falls. No bodies of water, such as pools or spas, were present on site.

According to the licensee, she provides food and snacks for the children. LPA reminded the licensee about the importance of sanitation and proper food preparation. According to the licensee, the children are required to bring their own food. Food and snacks were observed to be available for the children.

The sleeping/resting area was clean and sanitary. LPA observed playpens, sleeping mats, sheets, and blankets in the sleeping/resting area. LPA observed playpens to be free of loose articles and materials.

LPA reviewed five children’s files and confirmed that all five children have complete files that include their emergency contact and medical information. LPA found that the children's files did not contain their immunization records and the 15-minute sleeping logs for children under the age of two.



LPA reviewed the two licensees' files and confirmed that all required forms were present. LPA also found that the two licensees possess a current Pediatric First Aid/CPR certification, which expires on 11/2025. LPA also discovered that the two licensees did not complete their Mandated Reporter Training. LPA reminded both licensees that Mandated Reporter Training must be taken every two years. Mandated Reporter Training may be taken at www.mandatedreporterca.com.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

LPA informed the licensee to provide a copy of this licensing report dated 7/25/2024 that documents any Type A citations to the parents or guardians of all children currently enrolled by the next business day or the next day the children are in care and to the parents or guardians of any newly enrolled children for a period of 12 months from the date of this report. The deadline for providing this information is the next day, 7/26/2024. For the purposes of verification, the child's file needs to have either a signed Acknowledgement of Receipt of Licensing Report (LIC 9224) or another type of written statement.

The licensee was given the following forms in Spanish: LIC 126, LIC 627, LIC 700, LIC 995, and LIC 9224.

The licensee was given the following forms in English: a 15-minute sleeping log and an emergency disaster drill template, and LIC 9148

A $1000 civil penalty was issued to the facility for having two individuals without criminal record clearance.
