25 | On 8/2/2024, Licensing Program Analysts (LPAs) Leong and Alvarado conducted an unannounced Plan of Correction (POC) visit to the Family Childcare Home (FCCH) listed above. The visit was to follow up on the Type A deficiencies cited on 7/25/2024. LPAs met with the Co-Licensee, Juan Alvarez, and explained the purpose of the visit. The second licensee, Rina Santizo, was not present during today’s visit. According to Mr. Alvarez, the second licensee is at a doctor’s appointment.
Eight children (two infants, three pre-k, and three school-aged) and two adults (the co-licensee and the co-licensee’s son) were present during the visit. The two adults received criminal record clearance from the department. The licensee holds a large license and is within capacity limits and ratios.
LPAs observed a child safety latch on the garage door, making the cleaning chemicals discovered during the visit on 7/25/2024 inaccessible to the children.
LPAs received a written statement from Mr. Alvarez stating that the individual without a criminal record clearance on 7/25/2024 will not work at the home daycare unless the individual receives criminal record clearance.
The deficiencies, CCR 102417(g)(4) and HSC 1596.871(c)(1)(A), cited on 7/25/2024, will be cleared during today’s visit.
A copy of today's report was given to the Co-Licensee, Juan Alvarez
A Notice of Site Visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Co-Licensee, Juan Alvarez.
