Given the supporting details that were provided by the reporting party, the department was unable to determine whether these allegations were true.
Based on LPA interviews and observations, the two allegations, licensee hitting and threatening day-care children, may have happened or are valid, there is not a preponderance of evidence to prove the violation did or did not occur; therefore, the above allegation is found to be unsubstantiated.
Appeal rights was discussed with the licensee
“The Notice of Site Visit” shall be posted for 30 days.
Failure to maintain postings as required, will result in an immediate $100 civil penalty.
A copy of this report, the “Notice of Site Visit” and the licensee's “Appeal Rights” were given to the licensee
