On October 2, 2025, at approximately 2:30PM, Licensing Program Analyst (LPA) Nicole Tran and Licensing Program Manager (LPM) Ali Zebila conducted a subsequent complaint inspection in response to the above complaint allegation. LPA and LPM met with facility representative, Michelle Keane, and explained the purpose of the inspection. Chief Executive Officer of North Marin Community Services, Cheryl Paddack, later joined the visit. There were no school age children present during the visit.

As part of the investigation, interviews were conducted, and all relevant documents were collected and reviewed. Although the above allegation may have happened or is valid, based on LPA’s interviews, observations, and record review which were conducted, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is determined to be UNSUBSTANTIATED at this time.

No deficiencies were issued today during LPA and LPM's visit.

Notice of site visit was given and must remain posted for 30 days. Appeal rights were provided and explained.
