On 10/6/2023, at approximately 12:54PM, Licensing Program Analysts (LPAs) Jonathan Tse and Hanson Leong conducted an unannounced annual visit at Oakview Preschool. LPAs were granted entry into the facility by Head Teacher Nancy Hellman. LPAs explained the purpose of the visit. Present at today's visit were 45 preschool age children and 10 staff members. The facility is in compliance with teacher-child ratios. All staff members present are fingerprint-cleared and eligible to work in a childcare facility. Director Lindsey Toomajian arrived during the visit.

LPAs and Head Teacher inspected the facility for any health and safety hazards. The facility has all required postings available in the main entrance to the building. LPAs observed the classrooms to be in clean and orderly condition, with age-appropriate toys and activities available for children. There is a 2A10BC fire extinguisher that is fully charged. There is a combination fire and smoke detector system in the facility. The first aid kit is complete and fully loaded with all materials necessary to perform first aid. Electrical outlets that are not in use were observed to be covered. LPAs observed a diaper changing station was not in hand's reach of a hand washing sink. LPAs requested director to move the diaper changing station next to an appropriate sink. Director agreed.

The outdoor play area has play structures with appropriate padding underneath. There are two gazebos and two large umbrellas used to provide shade for the children. There are age appropriate toys and equipment present in the outdoors play area.

LPAs reviewed six staff files and six children's files. LPAs observed children's files to be complete. All teachers are fully qualified. LPAs reminded director of requirement to acquire immunization records from an employee.

Children bring their own food to the facility. Facility appropriately maintains children's medication in its original packaging and separated in plastic bags. Children bring their own water bottles and refill them at the facility.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPAs verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The facility received the Action Level Exceedance (ALE) report from the Vendor on 12/17/2022. Test results for this facility that exceeded a higher level of parts per billion (ppb) allowed, 5.5 ppb. Facility had a plumber remove the affected drinking fountain's button and nozzle, rendering it inoperable. LPAs verified the removal of the button and nozzle during today's visit. LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Today's citation was cleared during the visit. A copy of today's report, a notice of site visit, appeal rights, and the Cleared POC letter were given. The notice of site visit must remain posted for 30 days.
