On February 11, 2025, Licensing Program Analyst (LPA) Nathan Garcia arrived at the facility to conduct an unannounced, licensee initiated case management visit at the preschool facility. LPA met with teacher, Jennifer Faulk and was granted access to the facility. LPA explained the purpose of the visit and Jennifer gave LPA a tour of the outdoor area and play structure. The purpose of the visit is to inspect the newly built play structure. There were 20 children and 8 staff members present in the facility during the inspection. Site supervisors, Viviana Martinez, Ana Zambrano-Avila and Area Director, Donje Fields arrived at the facility to join in the inspection.

On February 3, 2025, LPA Garcia received an email from area director, Donje Fields requesting to inspect the newly built play structure for the facility. The facility is located in a elementary school site, and the facility has their own indoor and outdoor area. The outdoor play area plans were forwarded to LPA with the full extent of the design. According to staff, they just need to remodel the sandbox and paint a bike track and more shade. The entire play structure is fully built and usable.

LPA Garcia observed the wooden and plastic play structure with 1 slide, climbing areas, covered with shade. The whole structure is on tan bark covering the whole ground to provide soft padding. The structure is low to the ground, age appropriate for the preschool aged children. According to Donje, the sign indicating the intended age group for the play structure is on back order but it will be installed once it arrives.

As of today's visit, the new play structure is approved to be utilized by preschool children.

No deficiencies were cited today under Title 22 Division 12 of the California Code of Regulations.

A copy of this report, and the “Notice of Site Visit” were given to the Director. A Notice of Site Visit must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and the report was reviewed by Viviana Martinez..
