25 | On August 13, 2026, Licensing Program Analyst (LPA) Van conducted an unannounced Plan of Correction (POC) inspection and met with Assistant Director Landy De Honor, who granted access to the center. At the time of the inspection, the assistant director and 8 teachers were supervising 24 infants.
During the inspection on July 16, 2026, LPA observed violations related to unsafe sleep practices. Cribs contained loose articles. LPA also observed one infant sleeping in a sleep sack and another infant sleeping in an oversized pajama set. Because the pajama was oversized, the infant’s legs could not reach the designated leg openings, and the excess fabric caused the garment to function like a sleep sack.
During today’s visit, LPA verified through observation and record review that the previously cited deficiency had been corrected. Cribs were free of loose items, and appropriate safe sleep practices were in place. The director confirmed that staff were retrained on safe sleep requirements. The center also submitted the training agenda, training materials, and staff attendance signatures documenting completion of the safe sleep training.
Based on today’s inspection, the deficiency cited on July 16, 2026, has been corrected, and the facility is now in compliance.
A Notice of Site Visit was issued and must remain posted for 30 consecutive days.
An exit interview was conducted, and the report was reviewed with Assistant Director Landy De Honor. A copy of the report was provided to the Assistant Director.
