25 | On January 18th, 2022 at 9:30am, Licensing Program Analyst (LPA) Kassandra Medrano met with Center Director, Ilsa Miller and Assistant Director, Criselda Thomas, for a pre-licensing inspection. On 10/25/2022, the facility was hit by a mud slide, which lead to temporary closure, facility has requested temporary operation at this site until they can return to original site. At this time, facility has been notified by the Muir woods park community association that they must relocate by or before summer.This is subject to change depending community center.The facility will operate as a combination center with preschool (ages 2 years to entry into first grade), toddlers (ages 18-36 months) and infants (ages 6 weeks preschool children (ages 2 years to entry into first grade). The facility was inspected today, indoor and outdoor for health and safety hazards. Indoor: The preschool classroom measures a total of 900 square feet allowing for a capacity of 25 children. The facility is equipped with a fire extinguisher, smoke and carbon monoxide detectors. There is a separate bathroom for staff. Isolation of ill children will be in bathroom B until parent arrives for pick up. There will be drinking water readily available for children indoors. Outdoor: The outdoor area is below and fenced, facility plans to allow 12 to 15 children down at once. LPA has requested cushioning below slide. Facility is aware that there needs to be direct supervision at all times for structures. This facility plans to provide Incidental Medical Services – IMS. LPA reviewed location of storage for medication and equipment/supplies. Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. |
