25 | On July 16, 2026, Licensing Program Analysts (LPAs) Van and Naves arrived at the family child care home to conduct an unannounced Plan of Correction inspection. LPAs met with the licensee, Adriany Rodrigues, and explained the purpose of the visit. The licensee granted LPAs access to the home. Upon entry, LPAs observed the children were napping. The licensee and two assistants were supervising 6 children in care (4 preschool-aged and 2 infants)
During the annual inspection conducted on July 1, 2026, LPAs observed deficiencies related to mandated reporter training and staff immunization requirements. The licensee’s mandated reporter training certificate had expired, and the assistant had not completed the mandated reporter training. Additionally, the licensee’s helper did not have documentation of her Tdap (pertussis) immunization on file or available for review.
A technical violation was also issued on July 1, 2026, regarding children’s immunization documentation. Each child’s PM 286 (6/95) form must be updated when the child is due to receive required immunizations after enrollment in the family day care home.
During today’s visit, LPAs verified, through record review and observation, that the licensee had corrected all previously cited deficiencies and the technical violation. The licensee renewed her mandated reporter training, and the assistant completed the required training. LPAs confirmed that the helper’s Tdap immunization documentation was obtained and made available for review. LPAs also verified that each child’s PM 286 form was updated to reflect the current immunization status.
Based on today’s inspection, all deficiencies and the technical violation cited on July 1, 2026, have been corrected, and the family child care home is now in compliance.
A notice of site visit was given and must remain posted for 30 days.
