25 | On 7/3/2024, at approximately 1:10PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an announced prelicensing visit at 405 Ash Street, Mill Valley, for a change of location. LPA met with Applicant, Solange Arantes da Silveira. LPA explained the purpose of the visit. Present during the visit was Applicant and no children in care. Applicant is the licensee of facility number 214005520.
Intended Day Care Areas: Bonus Room (School Room), Bedroom #3, Front Yard and Backyard (Deck and Patio).
Off-limits Areas: Bedrooms #1 and #2, Office, Living Room, Dining Room, Bathroom #1, Bathroom #2 (located in Bedroom #2), Breakfast Area, Kitchen, Laundry Room, Carport (in Front Yard) and Storage Room (located at side of home).
LPA inspected the home for any health or safety hazards. There is a fully charged 2A10BC fire extinguisher present. There are operational combination smoke and carbon monoxide detectors. There is an operational fire alarm in the home. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care. Applicant has acquired fire clearance.
The Bonus Room (School Room) is clean and free of debris. Per Applicant, day care supplies and materials are in the process of being moved into the new location. LPA requested that photos of the furnished Bonus Room be submitted via email once Applicant is finished moving in. Applicant agreed.
LPA observed Bedroom #3 to contain a refrigerator. Bedroom #3 will be used as a napping area. LPA discussed adding childproof locks to the refrigerator. Applicant stated that they understood. Per Applicant, mats and sheets will be provided by the facility. Children will bring blankets from home, and they will be kept at the facility. The facility will clean sheets weekly or as needed.
