On January 24, 2025, LPAs Garcia and Badger arrived at the facility for an unannounced case management visit per request by the licensee to add a day care room and bathroom.
LPAs met with licensee and explained the purpose of the visit. Present during LPAs visit included 5 children (2 infant and 3 preschool aged children) and licensees' assistant.

The licensee requested by phone on 1/14/25 to add a bedroom and a bathroom as a day care area. LPA advised her to send an updated facility sketch and letter of intent to the RO. As of today's date, the documents have not been received at the regional office but the licensee mentioned that it was sent on 1/22/25. Licensee states that she wants to swap one of the off limit bathroom to become a day care bathroom which will be closer to the bedroom and napping rooms. And the initial day care bathroom be labeled as off limits area.

All adults living and/or working in the home have fingerprint clearance on file. Hours of operation are Monday through Friday 7:30am to 5:30pm
Previous Day Care areas: Living room space (middle of the house), back room for napping, backyard and bathroom.
Previous Off Limit areas: Kitchen and bedrooms #1, #2 and #3, bathrooms, from the rest of the house.

New Day Care Areas: middle of the house (main day care area), hallway to the bathroom, bedroom #1 for infant napping only and back room.
Off Limit Areas: kitchen, bedroom #2, bedroom #3, bathroom #1 (located near the front of the home) Off limit areas are made inaccessible with child safety door handles and/or a child safety gate. Home is equipped with a fully charged fire extinguisher, multiple first aid kits and multiple dual smoke/carbon monoxide detectors. LPA observed that the added bedroom has a carbon monoxide detector and camera. LPAs advised the licensee to put locks on the sliding closet doors.

As of today's date, the requested bedroom and bathroom is approved to be used as part of the day care areas, mainly for napping.

No deficiencies were issued during today's visit. A notice of site visit was given and must remain posted.
