25 | At approximately 9:20AM, Licensing Program Analysts (LPAs) Felias and Florio arrived unannounced to conduct a Pre-Licensing visit and met with Staff Member, Jiahui Xu and Applicant Eric Liu. Applicant has applied for a Change of Ownership for an existing Adult Residential Facility identified as Caremax Inc, Idylberry (216801078). Facility currently provides care to adults with developmental and intellectual disabilities. LPAs conducted an Annual visit for licensed facility on 03/19/2024 and identified no areas of concern. For today's Pre-Licensing visit, LPAs were informed that all clients were currently out of the community attending Day Program.
LPAs conducted a physical plant walk-through. LPAs observed the following: The facility was found to be clean with all exits free from obstruction. Facility had emergency lighting. Facility is a two story building with 4 Client bedrooms, 2 staff rooms, 3 bathrooms, and common areas. LPA observed required postings including the CCL Complaint Poster and Personal Rights for Individuals with Developmental Disabilities. Facility has an Infection Control plan and Emergency Disaster Plan on file. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. Emergency foods were observed in facility garage. Toxins were observed to be stored inaccessible to clients. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for clients. Mattress pads were in place or available for client use. Hot water temperatures for all sinks were found to be within Title 22 Regulations of 105 to 120 degrees Fahrenheit. Facility received an approved fire clearance on 01/30/2024 for 6 Non-Ambulatory clients. LPAs confirmed that contents of the facility First Aid Kit were sufficient. Staff had current First Aid certification. Medication was observed to be centrally stored and secure. Client files were located in a locked cabinet. Facility's fire extinguishers were last inspected October 2023. Smoke detectors and carbon monoxide detectors were tested and operational. Component III was conducted with Applicant. LPA notified Applicant that once licensed, they will need to complete new Admission Agreements and Care Plans for all clients.
No Deficiencies or Advisories given during visit. Pre-Licensing completed. Facility is ready to be Licensed as an Adult Residential Facility.
LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicant of Status.
Exit interview conducted. Copy of report discussed and provided. Signature on form confirms receipt of documents. |
