25 | On 4/3/2025Licensing Program Analyst (LPA), Stephanie Vega-Gonzalez conducted an unannounced case management inspection. LPA had attempted to conduct two prior inspections on 03/07/2025 and 03/12/2025. LPA met with Director, Anita Holt. The Purpose of today's inspection was to conduct a Case Management Inspection regarding the out door play structure that was constructed on January 25, 2025. LPA was informed by Anita that children have not had access to the play structure.
On today's inspection LPA Vega-Gonzalez inspected play structure, Adventure Tower, and observed the structure to be sturdy. Play structure is located in the back of the facility's building, that passes through an elementary grass area. LPA was informed that all children are supervised during the transition from indoor to outdoor play. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The outdoor play area is fenced. Safe toys and play equipment are observed.
LPA observed that the slide on the play structure was not anchored to the tower of the playset. LPA was informed by Anita Holt that she had not anchored it, due to wanting the Department to inspect it prior to installing it. Director stated that the facility will have the slide anchored by 4/7/2025. Anita Holt stated she will submit images to the Department and a short video showing that slide is placed correctly as per the assembly instructions before children have access to play structure.
Exit interview conducted and report was reviewed with Director, Anital Holt. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, a deficiency was cited. Please see LIC809-D.
