25 | On December 19, 2024, Licensing Program Analyst (LPA), Yesenia Fierro conducted an unannounced Case Management inspection. LPA met with Director Regina Wolf, LPA conducted a tour of the facility, and a census was taken.
The purpose for the inspection was to follow up on an unusual incident that occurred on December 10, 2024, where Director Regina Wolf, received information from Parent 1 that her child (Child 1) informed her that Teacher 1 had grabbed her arm in a rough manner.
Today, LPA spoke with Mrs. Wolf who admitted that after conducting an investigation and reviewing video surveillance footage of the incident she determined Teacher 1 mishandled child 1.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, LIC 809 D).
LPA Fierro informed Director Regina Wolf this report dated 12/19/2024 documents one Type A citation, which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Fierro informed Director Regina Wolf to provide a copy of this licensing report dated 12/19/2024 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Reports (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
