25 | On 3/18/2024 Licensing Program Analyst (LPA) Yesenia Fierro conducted a Case Management Inspection regarding an incident that was self-reported to the department on 1/23/2024 regarding care and supervision involving children. LPA met with Director, Adaluz Ramirez, and explained the reason for the inspection. LPA conducted a tour of the facility, took a census, reviewed records, and interviewed staff.
The purpose for today's inspection was to discuss two incidents that occurred on January 10, 2024, and January 11, 2024., involving child 1, child 2, child 3 and child 4 inappropriately touching each other. On January 10, 2024, during the P.M. child 1 inappropriately touched child 3 while they were dancing. On January 10, 2024, A.M. child 2 and child 4 were playing on a play structure (bus), child 2 displayed inappropriate gesture towards child 4. On January 11, 2024, A.M. child 2 was on the slide and displayed inappropriate behavior towards child 4.
LPA conducted interviews with staff 1 and staff 2 who were present during the 2 different incidents. LPA determined that there was no lack of supervision but there was a violation of personal rights.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are to be cited: (see next page, 809 D). Licensee was provided a copy of their appeal rights.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100
