| On 10/22/2024 Licensing Program Analysts (LPAs) Xona Xayavong and Yesenia Fierro conducted an unannounced case management lead testing/exceedance inspection. LPAs met with Site Supervisor Nancy Xiong. The purpose of today’s inspection was to conduct a case management inspection and to notify the facility that there has been a lead exceedance referred to as an Action Level Exceedance (ALE) of over 5.5 parts per billion (ppb) in the water at the facility. LPAs conducted a tour of the facility inside and out and a census was taken.
Fresno North Regional Office received notification of water lead exceedance at the facility resulting from a recent water lead sampling test. Based on records reviewed from Forensic Analytical Consulting Services (FACS) and discussion with Director Marie Hicks it has been determined that there is an exceedance of the cafeteria kitchen faucet sample number 240407216 A of 6.1 ppb.
Facility has tested twice once on January 9, 2024, and a second time on September 4, 2024. LPAs conducted an inspection of the kitchen sink which has been put off limits. LPAs spoke with Director Hicks who informed LPAs that Maintenance Supervisor Steve Whitaker was in the process of setting up an appointment to install a filtration system, Director Hicks stated they expect to install the filtration system within the next month. Once filtration system is installed the outlet will be flushed and retested prior to use.
Facility currently provides water via water dispenser and children bring their own water bottles from home. Facility will continue to provide water to children via water dispenser.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, A type B deficiency is being cited: (see next page, 809D). Site Supervisor Nancy Xiong was provided a copy of the appeal rights. An exit interview was conducted with Site Supervisor Nancy Xiong.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for the next 30 days.
