25 | On 09/17/2024 Licensing Program Analyst (LPA) Miguel Herrera conducted an unannounced Case Management inspection. Today, LPA met with Master Teacher Jessica Speairs and toured the facility, and a census was taken. The reason of this inspection is to discuss an unusual incident that occurred on 09/04/2024.
Based on interview and records review, LPA Herrera confirmed, that while playing in the facility’s playground child #1 sustained an injury that required medical treatment. Per Master Teacher Speairs two children were playing in the seesaw while child#1 lost their balance and landed on the ground, child #2 then attempted to get off the seesaw and tripped and fell on top of child #1. Child #1 immediately complained that their left arm was in pain, therefore First Aid was promptly administered, and parents were immediately notified.
LPA Herrera inspected the seesaw and determined it was age appropriate, the seesaw was approximately 1 foot height and approximately 4 feet in length. The seesaw was located in the wood chip area of the playground, and it was determined that there was appropriate supervision while children were playing. This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, there is no deficiency cited during today's inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted with Master Teacher, Jessica Speairs. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days. |
