32 | S2 attempted to redirect the children by telling them not to climb; however, C1 continued climb the slide up to approximately five feet high, then turned around, lost balance, and fell from the slide.
Interviews also disclosed that although C1 had been advised not to climb, staff did not take additional steps to prevent C1 from climbing from the bottom of the slide, despite knowing this behavior posed a risk of falls and potential injury. Facility protocols expect staff to redirect children from climbing up the slide and instead have them use the stairs to safely access the slide.
This lack of intervention indicates that staff failed to provide a safe environment for C1, as they were aware that climbing up the slide posed a risk of injury.
Record review indicated that C1’s authorized representative took C1 for a follow-up evaluation at urgent care. C1 was discharged without hospitalization, and the authorized representative was instructed to closely monitor C1’s condition. Per medical record, child is to restricted from physical education for one week.
During this inspection, it was confirmed that staff failed to ensure that C1 was accorded with their personal right to a safe and healthful environment, resulting in C1 sustaining an injury while in care.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, the following deficiency is cited: (see LIC 809D).
Exit interview conducted and report was reviewed with Site Supervisor, Malik Turner. Malik was provided with appeal rights.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
