During the course of the investigation, LPA interviewed Complainant, Director, and staff. Based on the information obtained during the investigation, there is a preponderance of the evidence to prove facility has a lice outbreak, facility has rodents and facility is operating out of ratio; therefore, the allegation is substantiated.
