25 | On 10/21/2025, Licensing Program Analyst (LPA) Ka Vang conducted an unannounced case management inspection. LPA toured the facility, and a census was taken. LPA met with Andrea Ceja and Stephanie Segovia, Site Supervisors. Today’s inspection was to discuss an unusual incident that occurred on 10/16/2025, in which Child #1 (C1) sustained a fracture to C1’s left leg. This incident was reported to the Fresno Community Care Licensing Office on 10/17/2025 by Site Supervisor Andrea Ceja.
Per interviews, it was reported that during the children’s morning outdoor activity, some of the children were playing on the “Falcons Roost” play structure while other children were riding the tricycles. Staff #1 (S1) was supervising and had direct vision of several children in front of S1 as they were playing on the structure. C1 was one of the children that S1 had direct vision. Per interview, C1 was climbing and going down from the beanstalk climber multiple times and C1 was doing fine. On the second or third climbing, C1 instantly lost C1’s balance from the top of the beanstalk climber and fell on the sand. Per interview, C1’s fell instantly that S1 did not have sufficient time to catch C1 before C1 fell on the sand.
Per observed, the beanstalk climber was part of the Falcons Roost play structures, which C1 fell from the top of the beanstalk climber. During this inspection, LPA observed the Falcons Roost have multiple small play structures attached to it, such as the beanstalk climber, rainwheel, mountain climber, tube clamp and slide.
LPA reviewed pertinent documentation at the facility. LPA verified teacher-child ratios were being followed during time of the incident. LPA observed record of the play structure, it was age appropriate. Based on the information obtained, this appears to be an isolated incident, and staff members took appropriate measures to address the child's injury, following proper policies and procedures and no regulations were violated.
