On 06/04/24, Licensing Program Analyst (LPA) Martha De Haro conducted an unannounced case management inspection. LPA toured the facility inside and outside, and a census was taken. LPA met with Director Isabel Abundez. The purpose of today's inspection was to discuss two unusual incidents, which occurred on 04/29/24 and on 05/14/24 in which two different children were injured on the playground.

Regarding the incident that took place on 04/29/24, child #1 was playing near the cushioned play area when he tripped on the cement edge, falling on a plastic piece of play equipment, referred to by staff as the "balance beam." Child #1 was observed to have a small cut on his lip, which was bleeding. Staff attended to child #1 appropriately and the parent was called who later took the child to the dentist, who did not find any major injuries to the child. Per Director Ms. Abundez, she plans to request that facilities make the grass more level to the cement. She also removed the plastic balance beam from the play area.



Regarding the incident, which occurred on 05/14/24, child #2 was playing in the play yard when he began to run after his friends. Per staff, child #2 tripped on his own feet and fell on the cement, scraping his knee. Staff attended to child #2 and cleaned/bandaged the injury. The parent was called who took the child to the doctor, who did not find any major injuries to the child.

LPA interviewed staff and reviewed records. After conducting the case management inspection and interviewing the teacher and Director, it was determined that both incidents were isolated incidents and not a result of lack of care and supervision. Both injuries occurred in different areas of the playground. Staff took appropriate measures to address each child's injuries, following appropriate policies, regulations, and reporting requirements.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with Director Isabel Abundez. This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days.
