25 | On March 19, 2025, Licensing Program Analyst (LPA) Yesenia Fierro conducted an unannounced case management inspection. LPA toured the facility inside and outside, and a census was taken. LPA met with Education Coordinator Cynthia James. The purpose of today's inspection was to discuss an unusual incident, which occurred on 02/27/2025 in which child #1 ran out of the classrooms back door.
During today’s inspection, LPA Fierro conducted staff interviews, based on the information obtained on 02/27/2025, child #1 ran out the back door during lunch onto the playground. Staff #1 ran after child #1, child # 1 was refusing to return to the classroom when Staff #2 came to assist Staff #1. Per interviews conducted child #1 was not cooperating and refusing to return to the classroom.
Staff #1 and Staff #2 attempted to assist child #1 off the ground by each staff holding one hand and Staff #2 lifting the child by the back of his pants, but child dropped back onto his knees dead weight. A second attempt was made by Staff # 1 and Staff # 2 child #1 was held by the hand, lifted by the back of his pants and carried back into the classroom.
Per interviews conducted LPA determined there was a violation of personal rights.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D) Education Coordinator was provided a copy of their appeal rights.
