25 | On April 02, 2026, Licensing Program Analysts (LPA) Miguel Herrera and Anna Porter conducted an unannounced Case Management Inspection. LPAs met with Licensee, Lili Iniguez and provided interpretation services in Spanish. During the inspection licensee's 2 assistants were present at all times. The purpose of today's inspection was regarding an Unusual Incident that was self reported to the Fresno Childcare Regional Office, the incident occurred on 03/10/2026. The incident involved child #1 sustaining an injury to their elbow that required medical attention.
During today’s visit LPAs conducted interviews, conducted facility observations, and reviewed records. During interviews, LPA Herrera did not obtain any information suggesting an incident occurred at the facility. Additionally, video footage reviewed showed the child #1 in good spirits during pick up and could not determine whether the child was injured. Per records reviewed the child was in attendance during the alleged incident, however, through evidence obtained it wasn’t clear whether the alleged incident occurred during day-care hours. LPA Herrera reminded licensee about safety and supervision around the play structure. The Licensee is aware of child safety around the play structure. The licensee understands that children must be always supervised while using outdoor play equipment and following manufacture guidelines.
During today’s inspection LPA Herrera observed the facility practiced proper supervision while playing indoors. Furthermore, it was an isolated incident and Licensees' took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements.
Exit interview conducted and report was reviewed with Licensee, Lili Iniguez. Appeal rights were provided. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there are no deficiencies cited during today’s inspection. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
