32 | · Licensee’s Pediatric CPR and First Aid certification was completed through HSI Pediatric with Emergency Medical Services Authority stickers (EMSA) and expires on February 11, 2026.
· Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on March 17, 2022.
· Licensee completed the Mandated Reporter Training for Child Care Providers on February 3, 2024.
· SB 792 immunizations verified and on file.
· Licensee rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Licensee obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the living room, LPA observed comfortable furnishings, a flat screen television mounted on the ceiling, children size table and chairs, and a high chair. In the play room, LPA observed various games, toys, and books for the children. Children will nap in the play room on mats, Infants will nap in the living room on play yards. Applicant understands she is to supervise children at all times.
· Fenced side yard has sod for the children. LPA observed the yard connects to the backyard, where there are piles of wooden planks, uneven sodded areas, and exposed sheet material.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives and cleaning compounds are stored in bottom kitchen cabinets. Medications are stored in a top kitchen cabinet. The kitchen is made inaccessible via child safety gates.
· There are no bodies of water in the home or premises.
· There are two dogs that are kept in the garage. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Licensee states there are no firearms or ammunition in the home or premises.
· Advised licensee fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example.
· Licensee is advised at least one staff member with current training in pediatric first aid and pediatric CPR is to be on site at all times children are present.
· Licensee is reminded that any advertising (of day-care) such as business cards, flyers/posters, and/or signs must include facility number as per Title 22 Regulation "Advertisements and License Number" 102359 (a).(CONTINUED ON 809-C) |
