· Applicant completed the Mandated Reporter Training for Child Care Providers on August 14, 2024.
· SB 792 immunizations verified and on file.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the living room, LPA observed comfortable furnishings, a flat screen television mounted on the wall, children size table, chairs and furniture, wooden cubbies for storage, a high chair, a chalk easel, a play yard, and various games, toys, and books for the children. Children will nap on cots, infants will nap on play yard in the living room. Applicant understands she is to supervise children at all times.
· Fenced backyard will be inaccessible to day care children. Applicant states the unfenced front yard and porch will be used for day-care children. Applicant understands that she must have 100% direct visual supervision of the day-care children when playing outdoors in an unfenced area.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives and medications are both stored in top kitchen cabinets. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via safety latches.
· There are no bodies of water in the home or premises.
· There are two dogs that are kept on the right side-run of the home. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Applicant states there are no firearms or ammunition in the home or premises.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example. (CONTINUED ON 809-C) |
