does not work. Safe and age-appropriate toys, highchair, kitchen set toy, toy cars, couches, television, sleeping cots, rocking chair, and other toys in the living room. LPA observe the first aid kit stored on the upper shelf next to the fireplace. Applicant has a non-contact thermometer.
Bathroom: Toilet, sink and faucet were clean and operable. LPA did not observe shampoo, mouthwash, medications, perfumes, air freshener, nail polish and nail remover that will pose a health and safety risk to children in care. LPA observed razors, shampoos, and mouthwash inside the bathroom. During this visit, razors, shampoos, and mouthwash were made inaccessible to children in care. The outlets are covered with child safety plugs.
Licensee stated that when a child shows signs of illness while at the day care, the child is separated from other children in the dining room in the facility. During this visit, LPA also observed an aquarium on top of a small cabinet in the dining area. The cabinet contains food for the fish, and the cabinet has a child safety latch.
Backyard: The facility has no body of water in the small backyard. LPA did not observe body of water in the backyard. Per licensee’s spouse, the children do not go in the backyard.
At the time of inspection, Licensee is aware of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing on the day of the incident and/or 24 hours of incident by telephone, fax and/or in writing to the Department. Licensee is familiar with the Unusual Incident Report form, LIC624B. The report on unusual incident/injuries can also be mailed to unusualIncidentreport@dss.ca.gov
Review of records to be maintained: LPA reviewed with licensee the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. LPA reviewed five children’s files, licensee, and assistant’s files. Based on records review, child 1 and 4 have missing LIC627 Consent for Emergency Medical Treatment. Licensee and assistants have current mandated reporter training and pediatric CPR/First Aid training. Licensee has proof of immunization against measles and pertussis. Licensee and assistant have TB clearance on file. However, the assistant does not have measles and pertussis vaccination on file. Licensee was advised to send copies of assistant measles and pertussis vaccination.
Because the licensee, Maria Rosas rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). |
