25 | Licensing Program Analyst (LPA) Marybeth Donovan conduced an unannounced visit to conduct a Case Management - Health Checks visit and met with Tracey Lundy Office Manager. LPA explained the purpose of the visit.
LPA toured the facility in part to include Assisted Living (AL), lounge, bathrooms, kitchen, pantry, freezer, walk in cooler, dining room, laundry room and offices. Facility interior room temperature was 74 degrees Fahrenheit (F). AL apartment unit hot water temperature measured at 114.8 degrees (F).
At 1:45pm in the kitchen LPA observed a minimum of 2 day perishable food supply and 7 day nonperishable food supply. Fresh fruit of bananas and pineapple were observed. Kitchen staff consisted of Chef Manager, 5 staff, and 1 server. Staff were finishing lunch service. Lunch menu consisted of Carrot and Ginger Soup, Baked Ziti with Vegetables and Bayou Shrimp Sandwich.
At 2:00pm in AL, LPA observed 2 Certified Nursing Assistants (CNAs), 1 Health Navigator and 1 housekeeper. Laundry room was operational and 1 laundry room attendant present. Facility is providing direct care services for residents, housekeeping, food service, and property maintenance. Staffing levels stable. Activities today included exercise classes, walking activity, local shopping and church services.
LPA obtained Forest Hill Residents, Family & Staff Weekly Memo dated 12/292021, Mindful Scoop Memo for January 2022 and Menu for 12/30/2021.
No deficiencies cited during this visit per the California Code of Regulations Title 22.
